1. Must received orientation before receiving applications
2. Complete the following forms:
  • Application.
  • Record clearance (Criminal and Child Protective Services check). Must be completed by everyone in the home 18 years of age or older.
  • Medical clearance (to be signed by a physician); required for everyone in the home.
  • Bethany questionnaire (employment, education, and family info, children preferences, etc.).
  • Self study (detailed persona history).
  • Home inspection by licensing worker (must have carbon monoxide detector, windows must be large enough in bedroom for exit by family member).
  • Provide at least three references (non-relatives).
  • Local police check (you may go to your county police station and request your records, or fill out a Bethany form that we can send in for you).
  • Driving record (can be requested at you DMV, or fill out a Bethany form that we can send in for you).
  • W-2/Pay stub.

3. Foster Parent Training
  • Complete PRIDE (Parent Resource Information, Development, and Education) Training (view training dates)

4. Forms to be read and signed:
  • Verification of orientation and receipt of application
  • Parent/Agency agreement
  • Discipline policy
